Fee Schedule

Cost of Examination(s):
$20.00 application fee payable to the Indiana Professional Licensing Agency.
Cost of the examination payable to the CDR in the form of a certified check, cashier's check or money order.  See their website for the exact cost.

Cost of Endorsement:
$20.00 application fee payable to the Indiana Professional Licensing Agency.

Renewal of certification:
$20.00 every two years, due by December 31of even-numbered years. If certification has been expired for less than three years, there is an additional late renewal fee of $10.00.  If certification has been expired for more than three years, the total cost will be $40 along with other requirements.  Please contact our office if your license has been expired for more than three years..

Verification of certification status to another state:
There is a $10.00 fee plus credit card fees to verify your license by digital certification. Click here to begin the digital verification process.
