Instructions & Application for Licensure

Please note that licensure for a dietitian in Indiana is not mandatory. It is a title protection for individuals who wish to use the title "Certified Dietitian," "Licensed Dietitian" or the credentials "C.D." or "L.D."  You may practice dietetics / nutrition therapy services without state licensure but cannot call yourself a "Certified Dietitian" or "Licensed Dietitian" unless you are licensed with this state. A dietitian registered by the commission on dietetic registration may use the title “registered dietitian” and the designation “R.D."

However, dietitians working in licensed health facilities may be required to be licensed by the Board.  Contact the Indiana State Department of Health for more information. 

Application Requirements

Apply Online Now! Go to MyLicense.IN.gov and you will either need to create or login to your Access Indiana, single sign-on account. You will complete the online application and submit payment with a credit or debit card. Please allow 48 business hours for your application to be received and reviewed. You will be contacted by a customer service representative with details of what additional documentation is required to complete your application.

Application Fee - The fee is $20.00 and you may submit online or you may print the paper form and submit a check or money order to the Professional Licensing Agency. 

Effective immediately Due to statutory requirements regarding processing times for application fees, the Indiana Professional Licensing Agency will no longer accept more than ten (10) fees per individual check for the application or renewal of a dietitian license with the Medical Licensing Board of Indiana.  

If a check is received with more than ten (10) fees per check, it may result in the return of your check along with the applications or renewals to your program for correction.  Further delays in processing time, as a result of more than ten (10) fees per check, may also lead to potential late fees. We apologize for any inconvenience this may cause and appreciate your partnership as we continue to work together to issue licenses as quickly and efficiently as possible.

Positive Response - If you have answered any of the questions on the application “yes” you must submit a signed, written statement detailing the occurrence/situation, the outcome, date of occurrence, if it is a malpractice payment the amount paid in your behalf.  If applicable please submit copies of all court documents and/or arrest records. Letters from attorneys or insurance companies are not accepted in lieu of your statement.

Official Transcript - Submit official transcripts from your college and/or university from where you received your degree. Please have your official transcripts forwarded directly from the school to our office.

Name Change - Official documentation indicating any legal name change, if your name differs from that on any of your documents. Acceptable documentation is a copy of your marriage license or divorce decree.

Pre-Professional Experience

  1. If you are registered with the Commission on Dietetic Registration (CDR), please enclose or upload a copy of a current registration card that indicates the period of registration and expiration.

Criminal Background Check - Once your application is received and you have been notified you must complete your criminal background check. Please review the criminal background check process on our webpage.

Verification of Licensure - You must submit the Verification of Licensure Form to each state in which you hold or have held a license, certificate or registration in any regulated health profession. Please have any verification sent directly to the Indiana Professional Licensing Agency.

Examination - The Commission on Dietetic Registration (CDR) has contracted with the American College Testing Program (ACT) to administer the Registration Examination for Dietitians. The examination is computerized and currently given year round, Monday through Friday.
