Fee Schedule

Registered Nurses:

Cost of Examination:

• $50.00 application fee payable to the Indiana Professional Licensing Agency.
• $200.00 payable to the National Council of State Boards of Nursing in the form of a certified check, cashier's check or money order.

Cost of Endorsement:

• $50.00 application fee payable to the Indiana Professional Licensing Agency.

Cost of a Temporary Permit:

• $10.00

Renewal of license:

• $50.00 every two years, due by October 31 of odd-numbered years plus
• 25% of which will be deposited into the impaired nurses' fund

Renewal of expired license:

• Renewal fee plus $50.00 late fee.

Licensed Practical Nurses:

Cost of Examination:

• $50.00 application fee payable to the Indiana Professional Licensing Agency.

• $50.00 late fee plus renewal fee

Prescriptive Authority for Advanced Practice Nurses:

(Optional for licensed RNs who are also Nurse Practitioners, Clinical Nurse Specialists, or Certified Nurse Midwives)

Cost of Prescriptive Authority:

• $50.00 application fee payable to the Indiana Professional Licensing Agency.

Cost of Indiana State Controlled Substances Registration (CSR):

• $60.00 application fee payable to the Indiana Professional Licensing Agency.

Renewal of license:

• $10.00 every two years, due by October 31 of odd-numbered years.

Late Renewal Fee:

• $50.00 every two years, due by October 31 of odd-numbered years.
