Fee Schedule

Cost of Licensure/Certification by Examination:
$100.00 application fee payable to the Professional Licensing Agency.

Cost of Endorsement:
$100.00 application fee payable to the Professional Licensing Agency.

Application fee for a Temporary Permit:
$50.00 application fee payable to the Professional Licensing Agency.

Renewal of License or Certificate:
$100.00 every two years, due by June 30 of even-numbered years.

Renewal of License or Certificate that has been expired for up to three years:
$100.00 plus $50.00 late fee

Renewal of License or Certificate that has been expire for more than three years:
$100.00 plus $100.00 pursuant to IC 25-1-8-6.

Verification of License or Certificate to another state:
$10.00 to each state to which information is to be sent.
