Application fee for Examination or Endorsement Candidates: $150.00

Renewal of licensure fee: $100.00 every two years

Renewal of licensure that has been expired for up to four years: $50.00 late fee plus the current renewal fee of $100.00

Renewal of licensure that has been expired for more than four years: Please contact the board office for information.

Renewal of licensure that is in inactive status: $50.00 every two years

Temporary Permits and Limited Licenses: $50.00
