Plumbing Apprentice Application & Instructions

Go to MyLicense.IN.gov and you will either need to create or login to your Access Indiana, single sign-on account. You will complete the online application and submit payment with a credit or debit card. Please allow 48 business hours for your application to be received and reviewed. You will be contacted by a customer service representative with details of what additional documentation is required to complete your application.

It is your responsibility to submit all documents with your application

1.) Must be at least 18 years old

2.) Completed application which may be submitted online with the $10.00 application fee OR you may submit the paper application. You will need the approved school sponsor signature and license number and the supervising Journeyman Plumber or Plumbing Contractor name and license number. Should you apply online, you may upload this portion of the paper application directly in the application on the Attachments page OR submit the form via the License Update feature.

3.) Bureau of Apprenticeship Training agreement form from the Department of Labor. Should you apply online, you may upload this form directly in the application on the Attachments page OR submit the form via the License Update feature.

4.) Any positive responses on the legal question on the paper or onlin application will require a signed, written statement, including all related details such as: the violation, location, date and disposition. The final court order document of pending or finalized charges. If you were ever placed on probation or are still on probation, you will need to submit the order removing probation or setting terms of probation. Should you apply online, you may upload this documentation directly in the application on the Attachments page OR submit the documentation via the License Update feature
