Closing the Nonresident Pharmacy
If you are closing out you Indiana nonresident pharmacy registration due to closure of the facility or you are no longer shipping prescriptions to patients into the state of Indiana, then you need to submit the closing information in writing to our office along with your original registration card. This may be mailed to our office at the following address:
