Verification of Licensure to Another State
The state of Indiana's electronic digital certification service allow a licensee to download and email a verification to another state board for a fee of $11.22. This is an official statement of the basis of licensure and includes the practitioner's name, profession, license number, status and any other conditions/restrictions of the license. License cards may purchased through PLA Online Services.You may also print a free copy of your license by logging into MyLicense.IN.gov and selecting the "Print License" button on your Licensing Home Page.
