Change of Pharmacist in Charge or Corporate Officer
Indiana law requires that a nonresident pharmacy must notify the Board of Pharmacy within thirty (30) days after any change of corporate officer or pharmacist in charge.

To change a PIC on a Non-Resident License:
Notification may be made to the Board by e-mailing pla4@pla.IN.gov. Please include the Indiana registration number of the nonresident pharmacy along with the new individual's name, home state license number and the effective date.
The PIC for non-resident facility does not need to be a licensed pharmacist in Indiana.
