Pharmacy Board Renewal
Renewal notices are sent approximately ninety (90) days prior to the expiration date. License holders with valid e-mail addresses on file will be e-mailed the renewal notice. Those who do not have valid e-mail addresses on file will be mailed the license renewal notice; this notice is mailed to the address of record with the Board. The Board has no way of knowing whether or not a notice reaches its destination; therefore, when a notice has been e-mailed to a valid e-mail address or mailed, the duty of the Board has been performed.
Online renewal is available 24 hours a day, 7 days a week for most license renewals. Online renewal takes only minutes and your renewal will be processed and available for verification the next business day. As defined below in the "Documentation Required for a Positive Response," should you answer positively with an online renewal, you may now use the "Upload Attachment" feature at the time of renewal. If you do not have the documents at available at the time of renewal, please use the instructions below.
To renew your license by paper you must select a form below. Print and mail the completed form with the required renewal fee to the Indiana Professional Licensing Agency. The address is on the form and checks should be made out to 'Indiana Professional Licensing Agency'. If your license has been expired for more than three (3) years or the license renewal form you need is not available please contact the Board by email at pla4@pla.in.gov.

You may verify your license has been renewed by using the Search & Verify option; allowing 24 hours or 1 business day for your transaction to process and your license renewal to complete. Once your completed renewal has been verified you may order a license card using our Order of Print a License online service. Indiana law requires those engaged in the practice of pharmacy to display their license/registrations where they are practicing; therefore, you must either download the free license card or purchase an updated blue card to post.
Documentation Required for a Positive Response
If you answered “yes” to any of the questions regarding disciplinary actions, arrests, convictions, etc., when completing the renewal process, you must submit the following:
- Detailed statement describing the response, including location, date, and disposition; AND
- Official documentation regarding the event (copies of all court documents, disciplinary orders, treatment information, addictionology evaluations, etc.).
The Board reserves the right to request additional information to help them make an informed decision regarding your renewal. The Board may also request a personal appearance in front of them to discuss the response(s).
