Renewal Information

Massage Therapist Certificates expire May 15th every fourth year (2017, 2021, 2025)

  • Renew online now using Access Indiana Single Sign-on at MyLicense.IN.gov. 
  • Massage Therapist Certificates expire 5/15/2021.
  • Proof of Professional Liability Insurance is required to renew.
  • You may submit a copy of your certificate of insurance by uploading it when you submit your online renewal, by mail with your paper renewal form, via email to pla14@pla.in.gov.
  • If you don’t have a copy of your certificate of insurance at the time you submit your renewal application, you may go to MyLicense.IN.gov and use the “License Update” feature to upload a copy later.    
  • Continuing education is not required to renew for the 5/15/2021 Massage Therapist Certificate Renewal.

Renewal notices are sent approximately ninety (90) days prior to the expiration date. Certificate holders with valid email addresses on file will be emailed the renewal notice. Those who do not have valid email addresses on file will be mailed the certificate renewal notice; this notice is mailed to the address of record with the Board. The Board has no way of knowing whether or not a notice reaches its destination; therefore when a notice has been emailed to a valid email address or mailed, the duty of the Board has been performed.

Online renewal is available 24 hours a day, 7 days a week for most renewals.

Paper Renewal Documents

To renew your certificate by paper, you must select a document below. Print and mail the completed form with the required renewal fee and proof of insurance to the Indiana Professional Licensing Agency. The address is on the form, and checks should be made out to 'Indiana Professional Licensing Agency.' If the renewal form you need is not available, please contact the Board by email at pla14@pla.in.gov.

Required Documentation 

You are required to hold professional liability insurance in order to practice massage therapy in the State of Indiana. Methods to submit your insurance include: 

1. Online: At the time of renewal by uploading a copy of your certificate of insurance on the 'Attachments' page. 
2. Online: Not as a part of a renewal, but by logging into your account at MyLicense.IN.gov and using the 'License Update' feature from the left menu. 
3. Sent via email to pla14@pla.in.gov, either directly from you or your insurance agency. 
4. Sent via USPS to the Professional Licensing Agency, either directly from you or your insurance agency.
