An Active Indiana Nursing License is Required to Apply for a Compact License

Active Nurse License: Go to MyLicense.IN.gov and click Register a Person to begin a Nurse Licensure Compact (NLC) application. You will complete the online application and submit payment with a credit or debit card. You will be contacted by a customer service representative with details of what additional documentation, outlined below, is required to complete your application. 

Initial Nurse License: If applying for initial licensure you may complete the application for initial licensure and will be able to request the compact license on that application form. The methods to apply for initial licensure are by examination (to take the NCLEX) and endorsement (actively licensed in another state).  The nursing license must be issued before PLA can issue the compact license.  

Expired Nurse Licences: Expired Indiana nursing licenses must be renewed before submitting a corresponding Compact application. If your license has been expired less than 18 months, you can renew online at MyLicense.in.gov. If your license has been expired over 18 months, it must be renewed by mail with forms available at Renewal Documents.  

 

Required Documents for Compact Licenses 

Below are documents which an individual must submit prior to being issued a compact license. 

  1. Completed Application: Applications may be submitted online at MyLicense.IN.gov or completed by paper and mailed to our office.  The application must confirm Indiana as the Primary State of Residence (PSOR).  
  2. Application Fee of $25.00: Pay by credit or debit card for applications submitted online at MyLicense.IN.gov. If applying by paper/mail make checks or money orders payable to Indiana Professional Licensing Agency. All application fees are nonrefundable.
  3. Name Change Documentation: Documentation of any legal name change is required if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
  4. Positive Response Documentation: If you answer "Yes" to any disciplinary/background questions on the application, explain fully in a statement that includes all details. Include the violation, location, date and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s).
  5. Criminal Background Check: If you submit a paper application, a confirmation email advising you to do your criminal background check will be sent to the email address supplied on the application.  If the application is submitted online, you may start your criminal background check process after the receipt screen appears after submitting your online application and payment.  Applicants who complete a criminal background check prior to their application being received will be required to complete another criminal background check.
  6. Proof of Primary State of Residence: If your mailing address does not have Indiana as your Primary State of Residence (PSOR), you will be required to submit a copy of one of the following documents showing Indiana as your current PSOR:
  • Driver’s license with home address
  • Voter registration card with home address
  • W2 form declaring primary state of residence
  • Federal income tax return including state of residence
  • Military form no. 2058 citing primary state of residence
