ADDRESS CHANGE
If you hold an Indiana Respiratory Care license (other license types, please ensure you are contacting the correct agency: CNA and XT licenses need to submit their requests to the State Department of Health), you are required to notify the Indiana Professional Licensing Agency of any address changes within fifteen (15) days of the change. You may complete this update 24/7 by logging into your account at MyLicense.IN.gov. You may also submit your address change in writing to pla14@pla.in.gov or our office at:

NAME CHANGE
You may have the name changed on your licensure records by submitting a legal name change document (marriage certificate, divorce decree, social security card or other legal name change documents). You may submit your name change document by using our License Update feature 24/7 by logging into your account at MyLicense.IN.gov. You may also submit your name change document via email to pla14@pla.in.gov or our office at:

Your request to change your name should include your name as it is listed now, your new name, and your license number or social security number.
