Obtaining a License Card

You can print off a free copy of your license by clicking here.

To sign in, enter your primary license number (e.g. CG12345678) as the user name, and enter the last four digits of your Social Security number as the password. The next page will display your license information. To the left of the screen, there is a link saying “Order License Card.” Click on this, and then on “Continue” next to the license you want print. The drop down menu on the next page will allow you to select “Free Paper Certificate Printout.” Enter the quantity you want, and add to the shopping cart. The next page will display the certificate and license that are available for free printing. At the bottom of the page, the Total Fee should show $0.00 for the free copy. Simply click on the “Print” button, and you will have a free printed license card and certificate.

If you wish to purchase a blue card stock wallet card and wall certificate, they are available for purchase for a fee of ten dollars ($10.00) per copy. You can purchase a card and certificate by clicking here.

The next page will display the total fee for the card and certificate, at ten dollars ($10.00) per copy. The payment processor link will direct you to our payment site, where you can pay by credit card. You will receive your card and certificate in the mail within approximately 3-5 business days.

A digital certification is available for purchase for a fee of ten dollars ($10.00) per copy, plus an access fee. The digital certification is not a license card, but can be used to certify your licensure status in the State of Indiana. The digital certification is a quick and easy way to verify licensure in Indiana for other states (for purposes like application for licensure) and/or employers. You can either receive the verification by mail, or simply download the verification when prompted.
