Licensing Information

Applications and Instructions
For State Specific Exam: completed applications must be postmarked two weeks prior to the scheduled quarterly state exam.  The state exam occurs on the same day as the scheduled Board meeting.  Please see the posted meeting dates for the next available exam. 

Effective December 1, 2009, the Indiana Professional Licensing Agency no longer prints and mails license cards as a courtesy upon licensure, certification, renewal or reinstatement. Visit the PLA Online Services webpage to order a pocket card or wall certificate.
