The ISDH Consumer Services and Health Care Regulation Commission is responsible for programs in the areas of health care facility licensing and certification, healthcare quality, vital records, and weights and measures. Through its programs, the Consumer Services & Health Care Regulation:*
- Promotes health care quality
- Licenses and certifies health care facilities, agencies, clinics, centers, and providers
- Ensures accurate state vital records and statistics
- Ensures accurate state standards for weights, measures and metrology

Report a Complaint, Incident or Crime to the ISDH
Report a complaint regarding a health care facility
Any individual may submit a complaint to the ISDH about care provided at any licensed or certified Indiana health care facility or entity. The ISDH investigates all complaints. The above link is to a page that provides information about the complaint process and how to report a complaint. A Complaint report form is provided for individuals to assist in the reporting of care issues.
Report an incident regarding a health care facility
State or federal regulations require health facilities or entities to report certain types of “incidents” to the ISDH. The above link is to a page that provides information about the incident reporting process and how a health care facility or entity reports incidents. An Incident report form is provided for health care facilities to report incidents to the ISDH.
Report a reasonable suspicion of a crime against a long term care resident
Federal law requires “covered individuals” to report a reasonable suspicion of a crime against a long term care resident to the ISDH and a local law enforcement entity. Covered individuals include an owner, operator, employee, manager, agent, or contractor of the facility. The included facilities include a nursing home, hospice, or intermediate care facility for individuals with intellectual disabilities (ICF-IID). The above link is to a page that provides information for long term care providers about the crime reporting requirements and how to report a reasonable suspicion of a crime against a resident. A Reporting a crime form is provided for individuals to report suspected crimes to the ISDH.
