Program Overview
Welcome to the home page for the Indiana State Department of Health (ISDH) Medical Radiology Services Program. The Program provides guidance and assistance to Indiana citizens and health care providers regarding radiation safety and protection. The Medical Radiology Services Program is responsible for the licensing and registration of medical radiology professionals and medical radiation sources in the State of Indiana. The Medical Radiology Services Program includes the following services:
- Licensing of radiology professionals (to include radiologic technologists, limited radiographers, nuclear medicine technologists, and radiation therapists)
- Radiation machine registration and compliance

To apply for a license for the first time in Indiana, please complete the Application for Radiology License. Please mail the application and payment to the address on the form. Currently, there is no option to apply online for first-time applicants.
To renew a license, we encourage everyone to renew their license online.
These are tips to renew a license online:
- You can renew your license and update your contact information at this direct link: https://www.in.gov/pla/3121.htm.
- You could enter your license number for User ID (which is case sensitive). Please make sure that you enter your license number with capital letters (i.e. X and T and zero, rather than the letter O), etc. You could enter the last 4 digits of your social security number for the password.
- Another option is to go to “Register” and enter only two of the fields (i.e. social security number and birth date).
- Another option is to go to “Renew License” on the left side of the screen, which is under “Menu”. If that doesn’t work, you should get out of the licensing home page and then try to register again. Then click on “Renew License”. It should work okay then.
- If you're not able to renew your license online, you may send your renewal form and payment to this address:

Renew or Verify a License or Registration
- All licensed individuals (Nuclear Medicine, Radiation Therapy and Radiologic Technologists) may renew their licenses online through the License Express online licensing system
- Online renewal is now available for individuals that hold licenses in multiple categories (e.g. Nuclear Medicine and Radiologic Technology). Please pay for each separately. Notify our office at MedicalRadiology@isdh.IN.gov that you have renewed more than one license. We will initiate the process for refunding the appropriate fees.
Address Change: If you have moved please notify our office of your new address. This will ensure that your license renewal reminder and license are delivered to you in a timely manner. You can change your address with either of the following options:
- online at http://www.in.gov/pla/license.htm click on Renew License and Update Address.

- US Mail addressed to:

Reporting a Complaint
To file a complaint about an individual that is operating a radiation producing machine or a medical practitioner regarding their employment of a radiographer or the use of their radiation producing machine(s) mail it to:
