Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.
Facility Health Data Center
Indiana’s Medical Error Reporting System requires that hospitals, ambulatory surgery centers, abortion clinics, and birthing centers report any reportable event as defined by the rules that occurs within that facility. If you are a hospital, ambulatory surgery center, abortion clinic, or birthing center enter the reporting system to report an event here.
